Now `Datetime` support `Datime(6)` to indicate the time scale. For different database , the max scale is not different.
`Datetime` means `Datetime(0)`.

Fix mssql issue with duplicate columns.
The `GetColumns()` method for the mssql dialect can return the same
column multiple times if the column is in multiple indexes.
